Countryside Health Center is a 62 bed skilled nursing facility, accepting Medicare, Private Pay, Insurance, & Medicaid. We have private, semi-private, and multiple bed rooms. Our residents enjoy activities hosted by our certified activity director. Countryside Health Center has a full staff of Certified Nurses Aides, as well as, a certified Social Services Director. The family and friends of residents at Countryside Health Center can be assured their loved ones will receive the highest quality medical and nursing services at an affordable price. Our programs emphasize total care to preserve quality of life, recognize all residents’ rights, and maintain the dignity of each individual.
